This form was created to allow members of industry to provide feedback on the various
Product Data Sheets (currently 55) that comprise the SBInet toolbox. Each Product
Data Sheet contains a number (average is 69 per PDS) of performance parameters,
each with threshold vale and objective value.
How to use: Select a product from the list to view the performance
parameters for that particular product. Use the checkboxes to the right of each
performance parameter or the “Select All” checkbox to
select which parameters for
which you'd like to provide feedback. Press the “Submit”
button below to go to the feedback input screen. On the feedback input screen, one may enter their name and
associated contact information (note that a “cookie” is stored to preserve this
information for feedback on multiple products). One may provide comments on the
parameter, description, threshold value, or objective value of the selected performance
parameters. At the bottom of the feedback input screen, one may recommend new performance
parameters that we might have overlooked. When all feedback for the selected product
has been entered, respondents should press the “Submit” button. Additional products may be selected for feedback from the list, and the process reiterates. Respondents
will receive a confirmation email containing the data they entered. Note that this
email is merely a courtesy and does not guarantee that the feedback will be incorporated
in the Product Data Sheet. |
